Pyoderma gangrenosum is a rare inflammatory skin disease of unknown etiology that belongs to the spectrum of neutrophilic dermatoses. It is destructive, has a variety of clinical manifestations, especially cutaneous, difficult to diagnose, chronic evolution, with frequent exacerbations and remissions; and there is no effective treatment of choice. Approximately 50 to 70% of cases are associated with systemic processes of various kinds. It is presented a 77-year-old male patient with a health history, who suffered trauma into the right foot dorsum, with a lesion that grew to become an ulcer as the days went by. Despite multiple treatments, the injury worsened. The patient attended to the dermatology service of Camilo Cienfuegos General Hospital in Sancti Spiritus, and a biopsy was indicated and the diagnosis of pyoderma gangrenosum was confirmed. The case was reported, because despite being a rare disease, it can occur in our patients, so it is necessary to think about it in order to diagnose it in a timely manner.
